Appointments

We are currently accepting new patients, however, we do require a referral and insurance authorization, if needed, to be faxed over to our office from referring physician. Appointments will be schedule with physician that patient is referred to at the first available date unless stated otherwise. Urgent referrals must have an urgent insurance authorization, if needed, and will be scheduled within 96 hours. If you can't keep an appointment, please give us a 24 hours notice.

Medical Records

You may access your medical records on the El Camino Mycare Portal. If you have not signed up for Mycare Portal, please ask our staff for assistance.

In order to keep our files current, please inform us of any changes in address, telephone number, and/or insurance coverage as soon as they occur.

Laboratory and Imaging Center

We do our best to send the lab orders and imaging orders to the contracted facilities with the insurance. However, insurance plans do make changes to their contracted laboratories and imaging centers often. We have no control and cannot be responsible for their policies, services, and charges. Please check with your insurance policy to confirm the laboratories and imaging centers that they are contracted with.

Payment

Co-pays and deductibles are collected at the time of service as required by California Law Business Professional 1680U.

Payment can be made using the patient portal. If there are any questions regarding a statement, please contact our finance department directly at 650-339-5615.
